Heroes to Hometowns
June 1, 2008 was our 2 year anniversary for Heroes to Hometowns San Diego, and our 5 month anniversary of H2H California! American Legion's Chula Vista Post 434 and Adm. Moffett Post 881 have stepped up to assist our Heroes, our Wounded Warriors, whether remaining in the military or transitioning to civilian life.
The San Diego Heroes to Hometowns Project has a fine history in assisting those who need us. We have been able to assist approximately 200 injured and their families since we began in 2006.
We thank our community, and community leaders, for their commitment to make a difference in the lives of our Heroes.
Heroes to Hometowns is designed to welcome home Service Members who are severely injured. In creating a positive transition home, Heroes to Hometowns inspires community pride and establishes a support network for Service Members and their families so they can live productive lives.
San Diego's Heroes to Hometowns is not a single organization but a coalition of community resources.
Heroes to Hometowns is one element of the Office of the Secretary of Defense's Military Severely Injured Center, which provides 24/7 support to augment the Military Services' severely injured programs.

If you are a military veteran of OEF/OIF and you or your family needs assistance, or if you desire to provide some means of assistance for one of the veterans or their family, please contact the area Community Champion and Coordinator by our contact form.
San Diego's Heroes to Hometowns is prepared to:
- Organize a welcome home celebration
- Help secure temporary and/or permanent housing
- Assist in adapting a home or vehicle
- Find jobs and educational opportunities
- Create a carpool for hospital visits
- Coordinate other services as required.
The San Diego community can help provide:
- Childcare
- Financial support
- Transportation
- Entertainment options
- Counseling
- Spiritual support
- Family support
- and many other products or services.

USA Together will match resources to those needing assistance so there must be a matching resource and request.
Heroes to Hometowns gives a personal contact which may be better suited to emergent or many needs.
